Session 3: The CCBHC Model: Findings from the Field

Description

This one-hour session will provide the audience with an overview of the CCBHC model as well as highlight CCBHC efforts across the country.  The speaker will share innovative practices and partnerships that have resulted in better health outcomes and that support sustainability efforts as well as discuss some of the challenges experienced by grantees during the COVID-19 pandemic.

Renee Boak
Renee Boak, MPH, National Council, CCBHC Consultant for Clinics
Renee Boak, MPH, serves as a Consultant where she specializes in CCBHC implementation for clinics.  Renee provides consulting regarding the integration of mental health and substance use disorder treatment, implementation of primary care clinics in a community mental health setting, population health management, and sustainability.  Renee also has experience with implementing a Medication Assisted Treatment Expansion grant as well applying for FQHC look-a-like status.  Renee’s experience working in CCBHC programs has provided opportunities to engage in innovative practices, such as the development of a pilot program to support individuals who are managing diabetes in a community mental health setting, as well as engage in sustainability work through alternative payment methodologies.  Prior to joining the National Council for Mental Wellbeing, Renee worked for an organization in Oregon that participated in the 2 year demonstration period and was a recipient of a CCBHC Expansion grant.
